You can download our application here. Fill in the form online, then print it out, sign it, and mail or bring it to the Habitat office (Habitat for Humanity, 213 E. Kirkwood, Bloomington IN 47408). Please open the attachment to the form (click on paper-clip image on the left-hand side) for information on additional documents needed to process your application.
Please review the Apply for a Home web page to learn more about our selection guidelines. Please note that this Interest Form is NOT an application for a Habitat home, but provides us with your contact information so that we can mail an application to you.
